Results will be returned quickly and will give you the following information:
- Name of the licensee or registration holder.
- License or registration type.
- License or registration number.
- Date license or registration was originally issued on, when it was last renewed, and the expiration date.
- Whether or not the license or registration is under any discipline currently. Disciplinary actions may include reprimand, civil penalty, probation, suspension, revocation, and voluntary surrender. Public documents associated with disciplinary actions can be obtained by written request addressed to the Board of Pharmacy, Public Information Officer.
Concerning license numbers: The letters that appear before the license number are considered part of the number. Don't forget to include them when using the license number field above (such as RP-0000001).
